Adverse events are one method of measuring hospital quality; however, collecting these data is problematic. This article reports findings from a study conducted collect data about the occurrence of adverse events in the hospital setting using an instrument called the Shift Coupon. The results demonstrated the viability of tbe Shift Coupon to collect adverse events data and identified the most eommonly reported adverse events and nurse-reported causes of adverse events.
