Information from these Alerts may be relevant when designing or redesigning processes.

Institute for Safe Medication Practices Canada (ISMP Canada)
The Institute for Safe Medication Practices Canada (ISMP Canada) is a national nonprofit agency committed to patient safety and was established to collect and analyze medication error reports.

ISMP Canada’s website gives access to a number of support services, publications, and specialist tools in the “products and services” section of its website. The institute publishes monthly safety bulletins, such as this one on the Top 10 Drugs Reported as Causing Harm Through Medication Error. Other bulletins discuss potential errors and mix-ups involving specific drugs, or general medication safety issues.

WHO Collaborating Centre for Patient Safety’s Patient Safety Solutions offers nine patient safety solutions for use by WHO Member states.
