Some History

In Winnipeg, community dwelling older adults are at an increased risk of negative health related outcomes including risks related to dementia, risks of falls and fractures, malnutrition, medication related harms, placing older adults living in the community at risk for adverse events.

Literature suggests that 46-79% of recommendations made by day hospitals are implemented and the remaining unresolved issues become the domain of community-based health care providers, clients & caregivers.

Effective communication between all levels of heath care providers, family and patients is considered a key element to reducing adverse events for all patients. Since deficits in communication and information transfer at hospital discharge is well documented, this is an area requiring attention and improvement.
