The Project

What is the Patient Safety in Day Hospital Transition Project About?

An exploratory study intending to capture the perspectives of key stakeholders (clinicians, pharmacists, patients, caregivers, and policy makers) regarding the current process of discharge planning and communication between day hospital and community based providers.

How is the Study Being Conducted?
3 Phases:

  1. Conducting initial focus groups and interviews
  2. Chart reviews
  3. A final round of multidisciplinary strategizing session to provide recommendations for improving day hospital discharge procedures

Study results will be communicated to all interested health care providers, day hospital clients, caregivers, and regional/provincial policymakers.

What are the Anticipated Outcomes?

  • To establish a transition process whereby the safety and access to community based services is assured
  • To minimize the risk of adverse events occurring within the transition period from day hospital to community based care
