Patient Safety in Day Hospital Transition
Background Information
The Day Hospital is in a pivotal position in addressing some of the issues placing clients at risk for adverse events through interdisciplinary assessment and rehabilitation.
This project’s aim was to engage key stakeholders to clearly identify challenges to the discharge process within Manitoba and to work to develop potential improvements to the system to ameliorate the identified issues. The project is now complete - please see final report for full details.
