- Written Strategic Patient Safety action plan developed
- Continue to use the Monthly staff newsletter “The Pulse” to highlight patient safety issues
- Patient Safety week celebrated with various activities
- Conduct a yearly Failure Mode Effects Analysis.
- Continue with ISMP newsletter circulation to clinical areas
- Good Catch contest held
- Quarterly Patient Safety scorecards/reports to the Board
- Critical Incident Reporting/Occurrence Reporting policies in place
- Disclosure policy and CIRC committee in place
- Conducted FMEA exercises on new sterilization process and new NOR-MAN RHA Flin Flon Medical Clinic and plan to do another in Mental Health shortly
- Trained staff in Root Cause Analysis to promote system reviews of critical incidents and also incidents and near misses.
- Launched “It’s Safe to Ask” at all acute care and community sites
- Surgical Check list initiated
- Concentrated electrolytes double locked and removed from general stock
- Patient Safety education part of general orientation to all new staff.
- Conduct the Climate of Safety Survey every third year to evaluate progress in improving the organizations culture of safety
Infection Control
- Hand Hygiene education program in place with sessions delivered and is a large part of Patient Safety Week.
- Ongoing Hand Hygiene education
- SSI Team in place, working to reduce surgical site infections. Good results with prophylactic antibiotics being administered in a timely way.
Prevention of Adverse Drug Events
- Medication Reconciliation Team in place with roll out September 7th 2010
- Occurrence reporting data management program to track and trend all occurrence reports, include adverse drug events in order to make improvements.
