- Write ‘Risky Business’ article for Regional responder every second month highlighting patient safety changes implemented as a result of Occurrence Reporting.
- Conduct a yearly Failure Mode Effects Analysis.
- Continue with ISMP newsletter circulation to clinical areas
- Continue to discuss occurrence trends at Coordinating Team meetings
- Conduct Patient Safety Leadership WalkRounds.
- ‘It’s Safe to Ask’ campaign launch Jan 9, 2007
- Conduct the Climate of Safety Survey every third year to evaluate progress in improving the organizations culture of safety
Infection Control
- Actively involved with Safer Healthcare Now! Prevent Surgical Site Infections and Ventilator Associated Pneumonia.
- Yearly Regional mandatory hand washing education
Prevention of Adverse Drug Events
- Actively involved with Safer Healthcare Now! Prevent Adverse Events Drug Events through Medication Reconciliation (Western node Collaborative)
- Formed a Medication Systems Safety Committee to address medication safety issues and review the ISMP Quarterly Action Agenda to determine if Brandon RHA should make changes based on the experiences of other healthcare organizations.
Other
- Plan to start use of the SBAR (Situation, Background, Assessment, Recommendation) Communication Tool in January 2008.
