Culture of Safety

  • Participated in the Culture of Safety Survey conducted by York University and sponsored by MIPS. The results received in November 2007 will be analyzed and used to determine appropriate actions to promote a culture of safety in the ARHA.
  • Initiated annual celebrations of Canadian Patient Safety Week (2006: “ARHA Q-TIPS-Quality Tips for Improved Patient Safety Contest” and 2007: “ARHA Patient Safety Photo Contest”)
  • Provided CPSI Root Cause Analysis training for 22 regional managers November 1, 2007 to promote system reviews of critical incidents and also incidents and near misses. RCA Training was sponsored by Manitoba Health.
  • Initiated “It’s Safe to Ask” in all sites and programs, beginning in January 2007 and repeated during Patient Safety Week October 2007 with support of MIPS.
    Conducted annual Failure Mode Effects Analysis: 2007 topic: Shift in Service from Acute Care to Transitional Care
  • Continued to use the weekly staff newsletter “FYI” to highlight patient safety issues
  • Provided patient safety updates to the Board
  • Reviewed the MCHP report re: Quality of Care Indicators in Long Term Care.
  • All Critical Incidents are investigated and reviewed using the Root Cause Analysis process.
  • Distributed quarterly analysis of Incident Reports to all Program/Area Managers

Infection Control

  • Provided mandatory hand hygiene training to staff.
  • Developed a more robust infection control data base to assist with better surveillance and response to infection
  • Provided ongoing staff and community immunization programs with emphasis on annual influenza campaign

Prevention of Adverse Drug Events

  • Participated in the Safer Healthcare Now! Medication Reconciliation project at all acute care sites
  • Utilized the ISMP Medication Self-Appraisal process to review current medication practices in the region (2005 and 2007).
  • Distributed the ISMP medication safety newsletter and the ISMP Nurse Adviser to all clinical staff
  • Used the regional Incident Reporting and Investigation process to address medication variances and issues.

Training and Education

  • Provided mandatory training/certification for ACLS, TNC and NRP for all nurses working in the acute care sites that offer these services.
  • Provided ASIST training to staff in the region
  • Provided Disaster code training in all sites
  • Developed a Falls Prevention and Management program: training to take place in 2008.
  • Provided training to managers and facility coordinators re: Disclosure of Adverse Events with Dr. Robson and Marilyn Kilpatrick presenting
  • Provided an opportunity for the Advisory Council to review and rate patient safety brochures
