<p><em>Safer Healthcare Now! </em>in partnership with the Canadian Patient Safety Institute, Victorian Order of Nurses for Canada and the Institute for Safe Medication Practices Canada invite you to participate in a <strong>Medication Reconciliation in Home Care Virtual Action Series</strong>.  This series is designed to support home care organizations improve medication safety through medication reconciliation.  Participants will attend five learning sessions over a three month period and be supported by faculty between sessions.</p>

Bienvenue! La campagne <em>Des soins de santé plus sécuritaires maintenant! </em>en partenariat avec l&#8217;Institut canadien pour la sécurité des patients (ISCP), les Infirmières de l&#8217;Ordre de Victoria du Canada et l&#8217;Institut pour l&#8217;utilisation sécuritaire des médicaments du Canada (ISMP Canada) vous invitent à participer à la Série d&#8217;apprentissage virtuel (SAV) sur le Bilan comparatif des médicaments en soins à domicile. Cette série est conçue pour apporter du soutien aux établisements de soins à domicile afin d&#8217;améliorer l&#8217;utilisation sécuritaire des médicaments par le biais du Bilan comparatif des médicaments. Les participants assisteront à cinq séances d&#8217;apprentissage étalées sur une période de trois mois. Des experts apporteront du soutien entre les séances.<br />
 <br />
Les résultats du projet pilote sur le Bilan comparatif des médicaments en soins à domicile de la campage <em>Des soins de santé plus sécuritaires maintenant</em>! (2008-2009) ont démontré que parmi les 611 clients sélectionnés pour faire l&#8217;objet d&#8217;un Bilan comparatif des médicaments, 45.2 % (275) avaient au moins une divergence nécessitant une clarification par un prescripteur autorisé. Les cliniciens ayant participé à ce projet pilote ont pu éviter des événements indésirables potentiels.<br />
Joignez-vous à cette série d&#8217;apprentissage virtuel afin de nous aider à faire déployer le Bilan comparatif des médicaments en soins à domicile partout à travers le pays.<br />

<p>There are Word versions of the placemats with a text box that allows insertion of text for patient safety champions who wish to use this resource but add their own patient safety tips. <p>Organizations can print their own quantities. These placemats can be used on patient hospital trays, in hospital cafeterias and in restaurants willing to promote the week in their communities. The placemats could be handed out at displays and visitors to the display asked to write down their questions and comments. These completed placemats could be displayed in busy people traffic areas in healthcare facilities to increase awareness and dialogue around patient safety.</p>

<p>The <em>It&#8217;s Safe to Ask Medication Card</em> is for people to record medications. The purpose of the card is to</p>

<li>help healthcare providers and emergency responders know patients&#8217; current medications</li>

They will summarize what providers, staff and improvement teams have identified as challenges and strengths when it comes to implementing the Surgical Checklist. <p>CPSI is inviting organizations to consider applying for inclusion in this study. They are looking for 20 participating organizations to enroll in this project. The deadline for your application to CPSI is <strong>September 7, 2010.</strong> MIPS may consider contributing funds to the application fee.  Organizations interested in this funding opportunity should contact Laurie Thompson by <strong>August 20, 2010.</strong></p>
