With the increasing national and international focus on patient safety, the need for a broad forum for learning
has been identified. Canada’s Forum on Patient Safety and Quality Improvement will provide multiple learning streams with a national and international flavour. The program for the forum will provide opportunity for a variety of professionals to participate, including, but not limited to, physicians, nurses, pharmacists, health care providers, educators, leaders, researchers, and board members.A few of the exciting speakers you will see at this event are:

Call for Abstracts

The Canadian Patient Safety Institute invites you to submit an abstract for a poster presentation at Canada’s
Forum on Patient Safety and Quality Improvement being hosted in Toronto, Ontario at the Westin Harbour Castle on April 28-30, 2009.

With the increasing national and international focus on patient safety, the need for a broad forum for learning
has been identifi ed. Canada’s Forum on Patient Safety and Quality Improvement will provide multiple learning streams with a national and international fl avour. The program for the forum will provide opportunity for a variety of professionals to participate, including, but not limited to, physicians, nurses, pharmacists, health care providers, educators, leaders, researchers and board members.

With the key message of patient safety and quality improvement, the forum will:

Keynote Speakers

Jacqueline Marcell
Jacqueline Marcell was so compelled being the sole caregiver to her challenging elderly father and sweet but ailing mother (both with early Alzheimer’s not diagnosed for over a year), that once she figured everything out, she gave up her stalled career as a television executive to become an advocate for eldercare awareness and reform.

Jacqueline mixes practical advice and humor with a bottom line message that there can still be a good life after a diagnosis of dementia, if it is properly managed medically and behaviorally.

Rosalie Kane
Rosalie Kane is a Professor of Public Health at the University of Minnesota. Her talk will explore the tension between managing risk and ensuring quality of life, and will include discussion of value conflicts, ethical dilemmas, and the potential conflict between safety and protection versus freedom for those using long term care services.

Canadian Reporting and Learning System (Feb 03 2009)

Please join The Canadian Patient Safety Institute to provide your input on the Canadian Reporting and Learning System!

Critical incidents: the most serious subset of adverse events and they occur far too often in Canadian healthcare.

When a critical incident occurs in one jurisdiction, other provinces or territories are not made aware of the hazard, or the learning that results from analysis, because a pan-Canadian system with these characteristics does not currently exist.

The Proposed Solution: The Canadian Patient Safety Institute is proposing CAERLS to address this identified gap.

1. CAERLS would receive de-indentified reports of critical incidents from provinces and territories when they occur.

2. CAERLS would contain a surveillance structure is to identify emerging hazards and solutions globally.

These two streams of information would be analyzed and those hazards and solutions, which meet a predetermined criteria, would then be communicated to Canadian healthcare stakeholders.

CAERLS is focused on the reduction of harm to patients by providing valuable information on emerging risks and mitigation strategies to those responsible for delivering healthcare services in Canada.

CPSI has made several recommendations for a pan-Canadian system and, as part of their commitment to collaborate on this important initiative they are currently seeking input from stakeholders across the country to further inform the development of this proposed learning system through a series of consultation meetings.

CPSI hosting a consultation meeting in Winnipeg, MB on February 3rd, 2009 at the Delta Winnipeg from 1:00pm to 5:00pm with lunch starting at 12:00pm and would appreciate your participation.
