The Canadian Patient Safety Institute in collaboration with its partners, the Institute for Safe Medication Practices Canada and Saskatchewan Health, will be offering an Advanced/Train the Trainer Root Cause Analysis Workshop.
The Workshop will be offered to individuals for the purpose of building expertise and spread of the process of root cause analysis throughout their organization.
Expressions of Interest are now being accepted for the workshop and positions will be offered to individuals based on several criteria and a pre-determined regional distribution across Canada. All submissions must be received by February 11, 2008. The Workshop will be held in Toronto, Ontario from March 17 - 19, 2008.
